You can download the Pre-College
Registration Form and then mail, fax or bring it in with your payment. The
Form is in .pdf format and can be read by Adobe Acrobat Reader.
A document describing Housing Terms and Conditions is also available for download in PDF format.
You can also download the Wisconsin DPI
Scholarship Application Form, but the original signature
document must be mailed or brought in person. For more information, see
the scholarships page.
By Mail
The completed registration
form, along
with your tuition check, money order, VISA, Mastercard or
American Express
information, will reserve places in the classes you choose.

In Person
Registration can also be made in
person at the Pre-College Office, Room 75 on the 2nd Floor,
Monday through
Friday. Please call for hours.
By Phone
Registrations using VISA,
Mastercard or American Express for tuition payment may be made by
calling Pre-College Office at 414-847-3330.
By Fax
Registrations using a charge card for
tuition payment may also be made by faxing a copy of the registration
form to the MIAD fax number 414-847-3339, including completed
VISA, Mastercard
or American Express information. Please call after faxing to
confirm that
your transmission was received. DPI Minorities Scholarship Application
Forms may not be faxed.
